Assertive Community Treatment (ACT)
Mobile multi-disciplinary teams provide intensive treatment, rehabilitation, and supportive services for individuals experiencing multiple life challenges, such as homelessness, unemployment, addictive disorders, legal involvement, and/or significant medical conditions. ACT uses a team approach to give consumers ongoing individualized care in their homes, where they work, and in other settings in the community where support is needed. The goal of an ACT team is to reduce the amount of time spent in institutional settings and to help individuals become more integrated within their community.

Behavioral Health Outpatient Clinic

Our team of Masters Level Clinical Social Workers (LCSW), Licensed Professional Counselors (LPC), Licensed Marriage and Family Therapist, and Licensed Clinical Addiction Specialists (LCAS), along with our Intake and Care Coordinator offer the following services in our Raleigh and Cary office settings:

  • Assistance with Intake documentation
  • A comprehensive clinical assessment to identify your mental health needs and service recommendations
  • Psychiatric evaluation, medication treatment and monitoring with nursing support as needed
  • Individual, Group, Couples, and Family counseling for Seniors, Adults, and Youth ages 12 and up
  • Individual and Group services for adult clients dually diagnosed with substance use and mental health disorders
  • Linkage to, and coordination with, community-based resources, treatment centers, and primary care
  • Discharge Planning and Transition to community-based services
  • After-Hours Crisis Line

Our clinical approaches include Trauma Recovery, Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), Solution Focused Therapy, Mindfulness to promote Stress Reduction and Management, Structural Family Therapy, Psychodynamic Psychotherapy, and other Evidence-Based Practices.

Substance Abuse Intensive Outpatient Program

A 12-week individual and group-focused non-residential program for adults providing intensive outpatient substance abuse treatment and secondary mental health treatment. Transportation is also available.

Tailored Care Management

Tailored Care Management (TCM) is a new care management model that began Dec. 1, 2022, for eligible NC Medicaid beneficiaries. With TCM, eligible beneficiaries will have a single designated care manager supported by a multidisciplinary team to provide integrated care management to address the beneficiary’s whole-person health needs. Under TCM, beneficiaries will have a single care manager equipped to manage all the beneficiary’s needs, spanning physical health, behavioral health, pharmacy, long-term services and supports (LTSS), and unmet health-related resource needs.

Drop-In Center
A safe and supportive environment is offered to individuals in the community experiencing life challenges. Daily programs not only focus on mental health/addiction recovery counseling, but also on physical health education and support, as well as leisure activities, structured skills training groups, peer recovery services, and FHR’s creative arts program, Studio 35.

Psychosocial Rehabilitation

FHR’s staff members provide a therapeutic, psycho-educational, and skill building milieu to foster a wide variety of skill developments. Community support activities promote independent living and self-worth. Pre-vocational skills are taught on a daily basis, which focus on the development of positive work habits. Members are encouraged to participate in FHR’s creative arts program, Studio 35. PSR staff facilitate groups which focus on increasing symptom management, confidence, self-esteem, social relationships, and community integration.

Peer Recovery Services
Peer recovery services are offered by persons with lived experience, who help support individuals served by FHR become and stay engaged in the recovery process. These supports include one-on-one group therapy discussion groups, as well as wellness-based trainings.

Peer recovery services are inclusive within all FHR adult programming in North Carolina.

 

Studio 35
Studio 35 is a therapeutic arts program of FHR. Studio 35 enables individuals to further explore the arts, art history, coping skills, and self-expression. By integrating the arts as a therapeutic approach, Studio 35 positively impacts individuals served, staff, and the greater community. This therapeutic arts program helps individuals become more engaged in the community, reduces isolation, improves stabilization, and teaches both vocational and arts based skills.

Participation in Studio 35 is available through all of FHR’s programming in North Carolina.
