FHR began providing services as the first Community Residence in the State of Rhode Island in 1975, launched programs in Massachusetts in 1980, developed the first Continuous Treatment Team in Sussex County, Delaware, in 1990, and instituted its Neuro-Cognitive Behavioral Program in Bangor, Maine, in 1999.

FHR piloted the first Correctional Discharge Planning Program in Rhode Island in 2001. In 2004, FHR enhanced its variety of services to include a Single Clinical Home Model of mental health care in Chester County, Pennsylvania.
In the spring of 2005, FHR launched its educational division, now known as the Center for Professional Development. This department ensures the integrity of each of its treatment models by providing its staff with extensive training. In May 2007, Fellowship Health Resources expanded its service area to include the community of Wake County, North Carolina. In 2010, FHR's creative arts program, Studio 35, was founded. This program promotes self-expression through all forms of the arts, including visual art, music, horticulture, and other mediums.
In 2011, FHR's Office of Peer Recovery Services was established. This department, offered by persons with lived experience, helps support individuals served by FHR become and stay engaged in the recovery process. The supports offered include one-on-one and group therapy discussion groups, as well as wellness-based trainings.
Now celebrating over 45 years of service, FHR is proud to offer an extensive array of supportive programs, including specialized clinical care and comprehensive care coordination. The organization's 80 community-based programs are supported by 400 employees and offered in Delaware, Maine, Massachusetts, North Carolina, Pennsylvania, and Rhode Island. These programs include:
